메디케어 가입, 순서대로 준비하면 어렵지 않습니다

메디케어 가입은 단순히 보험 하나를 신청하는 과정이 아닙니다.

먼저 Part A와 Part B 가입 여부를 확인하고, 그다음 Original Medicare 또는 Medicare Advantage 중 의료 혜택을 받을 방식을 선택해야 합니다. Original Medicare를 선택한다면 Part D와 Medigap 가입도 검토해야 합니다.

개인 상황에 따라 가입 시기와 필요한 절차가 달라지므로, 기존 보험을 먼저 해지하거나 다른 사람의 가입 방법을 그대로 따라 해서는 안 됩니다.

1단계: 가입 자격을 확인합니다

메디케어는 일반적으로 65세 이상인 미국 시민권자 또는 자격을 갖춘 영주권자를 위한 연방 건강보험입니다.

65세 미만이라도 다음과 같은 경우에는 자격이 생길 수 있습니다.

  • 일정 기간 사회보장 장애연금(SSDI)을 받은 경우

  • 말기 신장질환(ESRD)이 있는 경우

  • 루게릭병(ALS)이 있는 경우

영주권 기간, 미국 거주 기간, 본인 또는 배우자의 근무 기록 등에 따라 자격과 Part A 보험료가 달라질 수 있습니다.


2단계: 나의 가입기간을 확인합니다

최초 가입기간

Initial Enrollment Period

대부분의 사람은 65세가 되는 시기에 처음 가입합니다.

최초 가입기간은 일반적으로 총 7개월입니다.

특별 가입기간

Special Enrollment Period

65세가 된 후에도 본인이나 배우자가 현재 근무 중이고, 그 직장을 통한 단체 건강보험을 가지고 있다면 Part B 가입을 늦출 수 있는 경우가 있습니다.

일반적으로 다음 기간에 Part B 특별 가입을 신청할 수 있습니다.

  • 해당 직장보험을 유지하면서 본인 또는 배우자가 계속 근무하는 동안

  • 고용 또는 해당 직장보험이 끝난 시점 중 먼저 발생한 때부터 8개월 이내

그러나 다음 보험은 현재 직장보험과 동일하게 취급되지 않을 수 있습니다.

  • COBRA

  • 퇴직자 보험

  • 마켓플레이스 보험

  • VA 보장

  • 개인보험

COBRA를 선택하더라도 Part B 특별 가입기간의 8개월 계산은 일반적으로 퇴직 또는 현재 직장보험 종료 시점부터 시작됩니다.

직장 규모에 따라서도 Medicare와 직장보험 중 어느 보험이 먼저 의료비를 지급하는지가 달라질 수 있습니다. 65세가 되기 전에 고용주 보험 담당자와 Social Security 또는 Medicare에 확인하는 것이 중요합니다.


일반 가입기간

General Enrollment Period

최초 가입기간을 놓쳤고 특별 가입기간 자격도 없다면 일반 가입기간에 Part A 또는 Part B를 신청해야 할 수 있습니다.

이 경우 다음 문제가 발생할 수 있습니다.

  • 의료보험 보장 공백

  • Part B 지연가입 벌금

3단계: 자동가입 여부를 확인합니다

모든 사람이 자동으로 메디케어에 가입되는 것은 아닙니다.

자동가입될 가능성이 있는 경우

4단계: 현재 가지고 있는 보험을 확인합니다

메디케어를 신청하기 전에 현재 보험이 무엇인지 정확히 확인해야 합니다.

예를 들면 다음과 같습니다.

  • COBRA

6단계: Part A와 Part B를 신청합니다

Part A와 Part B 가입 업무는 Medicare가 아니라 Social Security Administration에서 처리합니다.

Medicare 가입 후 Part D 또는 Medicare가 인정하는 처방약 보장 없이 63일 이상 지내면 나중에 지연가입 벌금이 발생할 수 있습니다.


10단계: Medigap 가입 여부를 결정합니다

Original Medicare를 선택했다면 Medigap을 추가하여 일부 공제액, 코페이 및 공동보험 부담을 줄일 수 있습니다.

일반적으로 Medigap을 가입하려면 Part A와 Part B가 있어야 합니다.

65세 이상이고 Part B가 시작된 첫 달부터 한 번뿐인 6개월의 Medigap 공개가입기간이 시작됩니다.

이 기간에는 일반적으로 다음 보호를 받을 수 있습니다.

  • 해당 주에서 판매되는 원하는 Medigap 플랜 신청 가능

Medicare Enrollment Is Easier When You Follow the Steps

Medicare enrollment involves more than submitting one insurance application.

First, determine whether you need to enroll in Part A and Part B. Then choose between Original Medicare and Medicare Advantage. If you choose Original Medicare, you may also need to consider Part D and Medigap.

Enrollment timing and requirements depend on your individual circumstances. Do not cancel existing insurance or follow another person’s enrollment process without confirming how the rules apply to you.

Step 1: Confirm Your Eligibility

Medicare is generally available to U.S. citizens and qualifying permanent residents age 65 or older.

You may qualify before age 65 if you:

  • Have received qualifying Social Security disability benefits

  • Have End-Stage Renal Disease

  • Have ALS

Citizenship or residency status and the Medicare work history of you or your spouse may affect eligibility and the cost of Part A.


Step 2: Identify Your Enrollment Period

Initial Enrollment Period

For most people, the Initial Enrollment Period lasts seven months:

  • Three months before the month you turn 65

  • The month you turn 65

  • Three months after the month you turn 65

For example, if your birthday is in July, your Initial Enrollment Period generally runs from April through October.

Your coverage start date may depend on when you enroll. Preparing before your birthday month may help prevent a gap in coverage.

Special timing rules may apply if your birthday is on the first day of a month.

Special Enrollment Period

You may be able to delay Part B if you or your spouse are actively working and you have group health coverage based on that current employment.

A Part B Special Enrollment Period is generally available:

  • While the qualifying current-employment coverage continues

  • For eight months after the employment or coverage ends, whichever happens first

COBRA, retiree coverage, Marketplace coverage, VA benefits, and individual insurance may not be treated the same as current-employment group coverage.

The eight-month period generally begins when employment or current-employment coverage ends, even if you elect COBRA.

Employer size may determine whether Medicare or the employer plan pays first. Verify the rules with the employer benefits administrator and Social Security or Medicare.

General Enrollment Period

If you miss your Initial Enrollment Period and do not qualify for a Special Enrollment Period, you may need to enroll during the General Enrollment Period.

Possible consequences include:

  • A gap in medical coverage

  • A Part B late enrollment penalty

  • A penalty for premium Part A

  • A separate Part D late enrollment penalty

Contact Social Security promptly if you believe you missed an enrollment period.


Step 3: Check Whether Enrollment Is Automatic

Not everyone is automatically enrolled.

You May Be Enrolled Automatically If:

You have been receiving Social Security or Railroad Retirement Board benefits for at least four months before turning 65.

In that situation, you may automatically receive Part A and Part B. A Medicare welcome package and Medicare card are generally mailed before coverage begins.

Automatic enrollment may also apply to some people receiving disability benefits or benefits because of ALS.

You May Need to Apply If:

  • You are not receiving Social Security retirement benefits when you turn 65

  • You are delaying Social Security retirement benefits

  • You did not receive an automatic-enrollment notice or Medicare card

  • You have Part A and want to add Part B

Do not assume your Medicare card will arrive automatically. Check your status approximately three months before turning 65.


Step 4: Review Your Current Insurance

Identify all existing coverage, including:

  • Your current employer plan

  • A spouse’s current employer plan

  • COBRA

  • Retiree insurance

  • Marketplace insurance

  • Medicaid

  • VA or TRICARE

  • Union coverage

  • Individual insurance

  • A high-deductible health plan with an HSA

Determine which coverage pays first. Canceling existing insurance too early may create a gap or prevent you from returning to that coverage.

Important HSA Warning

You generally cannot make new HSA contributions after Medicare coverage begins.

If you enroll in premium-free Part A after age 65, Part A coverage may be retroactive for up to six months. HSA contributions during a retroactive Medicare coverage period may create tax issues.

Before applying for Medicare, confirm when to stop HSA contributions with your employer, tax adviser, or HSA administrator.


Step 5: Prepare Your Information and Documents

You may need:

  • Legal name

  • Date of birth

  • Social Security number

  • Address and telephone number

  • Citizenship or immigration information

  • Current or former spouse information

  • Employment history for you and your spouse

  • Current health insurance information

If you had current-employment coverage, you may also need:

  • Employer name and contact information

  • Employment start and end dates

  • Group health plan start and end dates

  • Employer verification of coverage

  • Documents showing retirement or loss of coverage

Keep copies of every form, confirmation number, and submission date.


Step 6: Apply for Part A and Part B

The Social Security Administration—not Medicare—processes Part A and Part B enrollment.

You may apply:

  • Online through Social Security

  • By calling Social Security

  • Through a scheduled appointment at a Social Security office

  • Through the Railroad Retirement Board, if applicable

If you already have Part A and are losing job-based coverage, you may use the Part B-only enrollment process.

Before applying, determine:

  • Whether to enroll in both Part A and Part B

  • Whether to enroll in premium-free Part A first

  • Whether you qualify to delay Part B

  • Your requested coverage start date

  • When HSA contributions should stop

Verify that your existing coverage permits a penalty-free delay before postponing Part B.


Step 7: Verify Your Medicare Card and Effective Dates

Your Medicare card will show:

  • Your name

  • Medicare number

  • Part A effective date

  • Part B effective date

Confirm that:

  • Your name is correct

  • The Part A and Part B dates are correct

  • Your address is correct

  • Part B was not accidentally omitted or added

Contact Social Security promptly if anything is incorrect.

Protect your Medicare number. Share it only with trusted health care providers, Medicare, or an insurance organization or licensed professional you have chosen to work with.


Step 8: Choose How to Receive Medicare Benefits

Option A: Original Medicare

Original Medicare includes Part A and Part B.

You then decide whether to add:

  • A stand-alone Part D drug plan

  • A Medigap policy

  • Retiree, union, or Medicaid supplemental coverage

Unless you join Medicare Advantage, you generally receive benefits through Original Medicare.

Option B: Medicare Advantage

To join Medicare Advantage, you generally need both Part A and Part B and must live in the plan’s service area.

Before enrolling, verify:

  • Doctors and specialists

  • Hospitals and medical groups

  • Prescription medications

  • Preferred pharmacies

  • Referral requirements

  • Prior authorization requirements

  • Copayments and annual out-of-pocket maximum

  • Actual dental, vision, and hearing coverage

  • Coverage while traveling or staying in another state

You must continue paying the Part B premium while enrolled in Medicare Advantage.


Step 9: Choose Prescription Drug Coverage

If you select Original Medicare, you may join a stand-alone Part D plan.

Prepare:

  • Names of all medications

  • Dosages and frequency

  • Preferred pharmacies

  • Mail-order preferences

Compare:

  • Formularies

  • Drug tiers

  • Deductibles

  • Prior authorization

  • Step therapy

  • Quantity limits

  • Pharmacy networks

  • Estimated annual drug costs

Going 63 days or more without Part D or other creditable prescription drug coverage may result in a late enrollment penalty.


Step 10: Decide Whether to Buy Medigap

If you choose Original Medicare, Medigap may help pay certain deductibles, copayments, and coinsurance.

You generally need Part A and Part B to buy Medigap.

Your one-time six-month Medigap Open Enrollment Period generally begins the first month you are age 65 or older and enrolled in Part B.

During this period, insurers generally cannot:

  • Deny you a Medigap policy they sell

  • Deny coverage because of a pre-existing condition

  • Charge more because of a pre-existing condition

After this period, you may be denied coverage or charged more unless another federal or state protection applies.

Medigap cannot be used with Medicare Advantage.


Step 11: Submit Your Plan Enrollment

To enroll in Medicare Advantage or Part D, you generally need:

  • Medicare number

  • Part A or Part B effective date

  • Address and contact information

  • Selected plan information

Enrollment methods may include:

  • Medicare Plan Compare

  • The insurance company

  • The plan’s website

  • A paper application

  • 1-800-MEDICARE

  • A qualified insurance agent or broker

Before submitting, verify:

  • Full official plan name

  • Plan type

  • Coverage effective date

  • Monthly premium

  • Whether drug coverage is included

  • Effect on current coverage

  • Whether current coverage must be canceled separately

Avoid submitting applications for multiple plans without understanding which enrollment will become effective.


Step 12: Confirm Enrollment

Submitting an application does not always mean enrollment is complete.

Confirm that you receive:

  • Application acknowledgment

  • Enrollment approval

  • Member identification number

  • Plan card

  • Coverage effective date

  • Summary of benefits

  • Formulary and provider information

  • Premium payment instructions

Do not cancel existing coverage until you verify approval and the effective date of the new coverage.


Step 13: After Coverage Begins

Complete the following:

  • Select a primary care provider if required

  • Give your new insurance information to your doctors

  • Update your pharmacy

  • Confirm that regular prescriptions can be filled

  • Set up premium payments if necessary

  • Create an online member account

  • Review hospital, emergency, and specialist costs

  • Check prior authorization requirements

If you have ongoing treatment or a scheduled procedure, confirm coverage before the new plan begins.


Step 14: Review Your Plan Every Year

Part A and Part B are generally one-time enrollments, but Medicare Advantage and Part D plans may change annually.

Review:

  • Annual Notice of Change

  • Evidence of Coverage

  • Updated formulary

  • Updated provider network

  • New premiums and copayments

  • Changes to additional benefits

Medicare Open Enrollment generally runs from October 15 through December 7 each year. Changes generally take effect January 1 of the following year.

  • Did I confirm my enrollment period?

  • Did I check whether enrollment is automatic?

  • Is my current coverage based on active employment?

  • Can I delay Part B without a penalty?

  • Did I confirm when to stop HSA contributions?

  • Are my Part A and Part B dates correct?

  • Did I compare Original Medicare and Medicare Advantage?

  • Did I verify my doctors and hospitals?

  • Did I compare all prescription medications?

  • Did I review the Part D penalty rules?

  • Did I identify my Medigap Open Enrollment Period?

  • Did I wait for approval before changing existing coverage?

  • Did I save all forms and confirmation records?
